DB8 Parental Consent Form

Hey guys, Graeme from Calgary here.
Just wondering where I could find the parental consent form for people under 18? And is it like I'm not racing if I bring the consent form to the race, but didn't mail before hand in time?

Sorry, anybody have a link or something to the waiver. I'm not sure where it is. I think it came in the email, but that was cleared in my inbox after a while.
